A podiatrist's guide to sizing children's shoes correctly — the most overlooked factor in Sever's Syndrome and growing-pain prevention.

Children's feet grow in spurts, and shoes that fit perfectly two months ago can be a half-size too small today. Tight or too-large shoes both contribute to Sever's Syndrome, heel pain, and growing-pain complaints.

Measure standing — not sitting — and re-measure every 8–12 weeks for kids 4–10, and every 4–6 weeks for kids in growth spurts. Allow about a thumb's width of room beyond the longest toe (which may be the second toe, not the big toe).

When a child complains of heel pain after sports, the first place we check is the shoe. The right size with proper heel support resolves a remarkable percentage of Sever's complaints without further treatment.
